Participant Eligibility

Those individuals who have the following criteria may be eligible to enroll at El Toro Adult Day Services:

• One or more chronic or post acute medical, cognitive, or mental health conditions

• Limitations in performance of two or more activities of daily living and needs assistance or supervision

• Insufficient support to maintain independence in the community

• A high potential for further deterioration and subsequent hospitalization or institutionalization

• Conditions that require adult day health care services such as nursing, personal care/social services, therapeutic activities, etc.

 

Services Provided

Services provided at El Toro Adult Day Services include but are not limited to:

• Medical & Nursing Care

• Social Services

• Physical Therapy

• Occupational Therapy

• Exercises

• Recreatiional Activities

• Caregiver Support

• Health Education 

• Hot Meal & Snacks

• Transportation To and From the Home

Admission Procedure

A multidisciplinary team of health professionals conducts a complete assessment and develops an Individualized Plan of Care (IPC), which determines the medical, social, and psychological needs of each and every participant.

The IPC is reviewed with the participant and/or caregiver to involve them in promoting quality services.

Cost for ADHC Services

Adult Day Health Care services are covered by Medi-Cal or private payment:

• Medi-Cal: If the potential participant is eligible for Medi-Cal, the center will obtain prior authorization from the enrolled Medi-Cal Managed Care health plan or local Medi-Cal field office to provide services.

• Private Pay: Private pay participants are charged based on the center's private pay rate.
